When a patient enters the Seasons Hospice care program, they will be assigned a team of compassionate and knowledgeable professionals that guide and support them every step of the way. Care teams are made up of registered nurses, physicians, chaplains and spiritual support personnel, volunteers, personal care aids, and bereavement counselors.
RN’s will visit patients wherever they call home, whether that be a hospital, nursing home, or residential home. Our nursing services include delivering palliative medicine to ensure that patients are as comfortable as can be.
At Seasons Hospice, we have several geriatric and palliative care physicians that are ready to form relationships with patients, but we also understand that many patients will want to continue their relationship with their personal physicians. Either way, our Seasons Hospice staff will coordinate care to ensure that your loved one receives their choice of medical care.
